Case Manager

• Provide case management support to children, young people and families.
• Assess needs and develop support plans to achieve positive outcomes.
• EOI role based in Western Sydney

This is an expression of interest to work for an organisation dedicated to protecting children and young people, supporting families, and creating positive change in the lives of children and communities. Our client is looking to recruit Family case managers for a child protection related roles in Western Sydney.

This role will involve;

• Providing case management support to children, young people and families.
• Assessing client needs, develop support plans and work collaboratively with clients to achieve identified goals.
• Providing information, referrals, advocacy and practical support to clients and families.
• Monitoring client progress, respond to identified risks and support the safety and wellbeing of children, young people and families.

The requirements for this role include;

• Tertiary qualification in Social Work/Welfare, or a relevant field.
• Demonstrated experience within the OOHC or Community Services Sector.
• High level of written and verbal communication skills.
• Knowledge of NSW Child Protection policies and legislation.
• A demonstrated experience in either child protection or OOHC Services.
