About our Client:

About the Role:
As a Support Coordinator you will be responsible for assisting clients in managing their NDIS plans, helping them achieve long-term goals, and fostering independence. The role involves collaborating with clients to review and manage their plans, identifying actionable steps, and coordinating supports tailored to individual needs. You will also track and report client hours, maintain accurate records, and provide administrative support for meetings and billing issues. The role may require travel and participation in ongoing professional development.
  
Key Responsibilities:
  • Conduct thorough assessments to understand participants' goals, strengths, and challenges.
  • Help participants understand their NDIS plan and align supports with their goals and budget.
  • Source and connect participants with suitable service providers, including healthcare and specialist support.
  • Advocate for participants’ needs with service providers, government agencies, and other stakeholders.
  • Assist in building participants' skills and independence through training and coaching.
  • Identify potential risks, develop safety plans, and manage crisis situations.
  • Maintain communication with participants, families, carers, and service providers to ensure coordinated support.
Essential Criteria:
  • Minimum Certificate IV in Disability Services or relevant similar qualifications.
  • Fluent in Auslan or Level 2 Basic Auslan is highly recommended.
  • Previous experience working in the disability sector; previously Support Coordinator experience will be highly desired.
  • Knowledge of the NDIS, NDIS policies and Disability Inclusion Act 2014.
  • Valid current driver’s licence and ability to travel across Townsville and outer areas.
